Tutorial: Create Your First EBRAINS Form
In this tutorial, you’ll learn how to create and share forms using templates. We’ll build a team feedback form from start to finish.
What you’ll build: A feedback form using a pre-built template
Time required: 10-15 minutes
Prerequisites: An EBRAINS account
Why Use Templates?
Templates save you time by providing ready-made forms for common use cases:
- Requirements gathering - Collect feature requests and needs
- Event registration - Manage attendees and RSVPs
- Feedback collection - Gather opinions and suggestions
- Survey forms - Conduct research and polls
Instead of building from scratch, start with a template and customize it to your needs.
Browse Available Templates
- Go to EBRAINS Forms Templates
- Browse the available templates
Select a Template
For this tutorial, let’s create a feedback form:
- Find the “Feedback Form (NPS)” template (or similar)
- Click “Preview” to see what’s included
- Click “Use Template” to start customizing. A toast will appear saying “Template cloned. It’s now your form. Edit fields, share, or delete it from My Forms.”
Customize the Form Details
You’re now in the form editor:
- Change the title: Update to something specific like “Team Feedback”
- Update the description: Explain what feedback you’re collecting
- Example: “Help us improve our team processes by sharing your thoughts”
- Set the button label: Change submit button text if needed (e.g., “Submit Feedback”)
- Choose button intent: Keep “Primary”
Configure Form Settings
In the left sidebar > advanced:
Visibility:
- Off (default) - Only EBRAINS members can access
- On - Anyone with the link can respond (public form)
- For team feedback, keep it Off (internal only)
Expiration date (optional):
- Set a deadline if feedback is time-sensitive
- Example: End of quarter, end of month
Submission policy:
- Off (default) - One submission per user
- On - Multiple submissions allowed
- For feedback, keep it Off (one response per person)
Customize Form Fields
Now edit the actual questions:
Review existing fields:
- Each template comes with sample questions
- Click on any field to edit it
Edit a field:
- Click the field you want to change
- Update the label (question text)
- Mark as required or optional
- Add help text if needed
Add new fields:
- Click "+ Add Field"
- Choose field type:
- Text Input - Short answers
- Text Area - Long feedback
- Select - Dropdown choices
- Radio Buttons - Single choice
- Checkboxes - Multiple choices
- Rating - Scale from 1-5 or 1-10
Remove unwanted fields:
- Click the trash icon on fields you don’t need
Preview Your Form
Before publishing, test the form:
- Click “Preview” (top)
- Fill out the form as a user would see it
- Check:
- All questions make sense
- Required fields work correctly
- The flow feels natural
Publish Your Form
When you’re satisfied:
- Click “Publish Form” (top right)
- Your form moves from Drafts to Live
- You’ll see a confirmation with your form link
Share Your Form
Copy the form link and share it:
- Email - Send to your team members
- Chat - Post in EBRAINS Chat channels
- Wiki - Embed in Collaboratory pages
The link format: https://forms.ebrains.eu/f/[your-form-id]
Tip: You can also add team members as collaborators:
- Go to My Forms
- Click your form
- Manage membership to add Editors or Viewers
Monitor Responses
As submissions arrive:
- Go to My Forms
- Click on your form to view submissions
You can also view all your submissions at My Submissions
Export Data
When you’re ready to analyze feedback:
- Open your form in My Forms
- Click the export icon (download button)
- Download (CSV) and analyze the data
